- PBSE is Part I of the ABCP's CCP process; CAPE is Part II, and passing PBSE alone does not award CCP.
- The exam has approximately 220 multiple-choice items, a four-hour limit, and a USD 360 fee (plus USD 100 if late).
- A scaled score of 400 on a 120-480 scale passes; the standard is criterion-referenced, not curved.
- All 11 knowledge-base domains may be tested, and no official percentage weighting was verified.
What the Perfusion Basic Science Examination Is
The Perfusion Basic Science Examination (PBSE) is the first of two exams in the Certified Clinical Perfusionist (CCP) pathway run by the American Board of Cardiovascular Perfusion (ABCP). The second is CAPE, a separate examination with its own four-hour timer. PBSE emphasizes basic-science processes: the "why" behind physiology, pharmacology, and equipment behavior. CAPE emphasizes application. Passing PBSE does not give you the CCP; the credential is awarded only after both examinations are passed.
If you are still orienting yourself, the explainer pages What Is PBSE? and What Does PBSE Stand For? cover the basics. This cheat sheet assumes you already know the name and want the facts compressed into one place.
Format, Fees and Registration at a Glance
| Item | PBSE fact |
|---|---|
| Issuer | American Board of Cardiovascular Perfusion (ABCP) |
| Role in certification | Part I of the CCP examination process (CAPE is Part II) |
| Items | Approximately 220 multiple-choice questions (not an exact count) |
| Time limit | Four hours |

Because the late fee and the no-refund policy are both real, treat the application deadline as a hard date. For the full cost picture, including costs beyond the exam fee, see PBSE Certification Cost 2026: Complete Pricing Breakdown.
Scoring: The 400 on a 120-480 Scale
The passing scaled score is 400 on a 120-480 scale. The standard is criterion-referenced, meaning it reflects a defined level of competence rather than your rank against other candidates. Two practical consequences follow:
- There is no grading curve. A strong cohort does not raise the bar, and a weak one does not lower it.
- There is no published raw-percentage rule. You cannot convert 400 into "you need X of 220 correct." Be skeptical of any source that claims an exact raw cutoff.
A preliminary pass/fail result is issued at the test center. Final verified results are provided within six weeks. The detailed breakdown lives in PBSE Passing Score 2026: Exactly What You Need to Pass.
Eligibility Cheat Sheet
Eligibility is where many applicants lose time, because the documentation requirements are specific. Normally you need:
- Graduation from an accredited perfusion program before testing (you may apply while enrolled, provided graduation precedes the examination date).
- An official graduation transcript.
- A school clinical-competency statement.
- A documented case record.
The Case-Record Rule
The detailed guidebook criteria take precedence over the simplified "75 CPB" wording that appears on the main PBSE page.
- At least 75 qualifying primary clinical perfusion activities, made up of at least 70 primary CPB cases plus five qualifying 3P ECMO or 6P VAD cases.
- At least 10 pediatric CPB cases performed or observed. Performed pediatric cases may overlap the 75; observations cannot count toward the 75.
- Cases must occur in the US, its territories, or Canada.
- An ABCP-certified CCP instructor must be physically present during CPB cases.
- Effective November 1, 2025, submitted records require the instructor's full first and last name and CCP number, including the overseeing CCP for ECMO/VAD cases.
Limited historic exceptions exist for pre-April 15, 1981 entrants and previously certified perfusionists, but most candidates will be on the standard path. The full walkthrough is in PBSE Requirements 2026: Eligibility, Prerequisites & How to Qualify.
The 11 Knowledge-Base Domains
ABCP publishes 11 knowledge-base headings shared by PBSE and CAPE. These are official examination headings, not an editorial study curriculum, and all of them may appear on PBSE. No official percentage distribution was verified, so this cheat sheet does not assign weights or name a "biggest" domain. The knowledge-base survey behind the headings was last conducted in 2023.
| # | Domain | What PBSE-style questions tend to probe |
|---|---|---|
| 1 | Anatomy & Physiology | Cardiovascular, pulmonary, renal and hematologic mechanisms underlying bypass |
| 2 | Pharmacology | Drug actions, interactions, and effects relevant to bypass |
| 3 | Pathology | Disease processes that shape perfusion management |
| 4 | Laboratory Analysis | Interpreting blood gases, coagulation and chemistry values |
| 5 | Quality Assurance | Process, documentation and safety systems |
| 6 | Devices & Equipment | How pumps, oxygenators and circuit components work |
| 7 | Clinical Management | Applying principles to conduct of bypass |
| 8 | Special Patient Groups | Populations that change management, such as pediatric patients |
| 9 | Special Procedures/Special Techniques | Procedures and techniques beyond routine bypass |
| 10 | Catastrophic Events & Device Failure | Recognizing and reasoning through emergencies |
| 11 | Monitoring | Parameters, interpretation and trends |
How to Use the Domains
Because PBSE leans toward basic-science processes, expect questions that ask you to reason from mechanism rather than recite a protocol. A pharmacology item, for example, is more likely to test why an agent behaves a certain way than to ask for a memorized dosing script. Domains 1, 2, 3 and 4 are where mechanism-heavy reasoning concentrates, though nothing officially limits where a mechanism question can appear. The domain-by-domain breakdown is in PBSE Exam Domains 2026: Complete Guide to All 11 Content Areas.
Block Delivery Starting Fall 2026
Starting with the Fall 2026 administration, the examination is delivered in blocks. The rule that changes your strategy: once you submit a block, you cannot go back to it. Total testing time and the approximate item count are unchanged, and no official block size has been published, so do not plan around a specific number of questions per block.
Key Takeaway
Practice committing to answers. Under block delivery, the habit of leaving hard items blank to "come back later" works only within a block, not across blocks. Rehearse flagging within a block and making your final calls before you submit.
The relaunched official ABCP practice exams use a block format, approximately half-length timing, domain feedback, and online calculator and whiteboard features. Those descriptions apply to the practice exams. They should not be read as a separately verified policy on what tools are permitted in the actual exam, so confirm testing-day rules with ABCP and Pearson VUE before you sit. You can also drill in a similar format with the practice questions at our main practice test site.
Dates and Windows
- Fall 2026 window: October 20-24, 2026 per the current ABCP calendar. The older 2025 Annual Report lists October 21-24; the current calendar takes precedence.
- Fall 2026 application: closed October 5, 2026, with a late fee applying after September 21. As of the October 14, 2026 research date, that window is no longer open for new applications.
- January and July 2027 pilot windows: limited to eligible retesting candidates and qualifying CAPE holdovers with prior-window applications on file. They are not open to unrestricted new PBSE applicants.
- Regular spring/fall windows remain the route for most first-time candidates.
Scheduling details and deadline planning are covered in PBSE Exam Dates 2026: Testing Windows, Deadlines & Scheduling.
Reading the 2025 Pass Data Correctly
The ABCP 2025 Annual Report publishes PBSE results by administration and by candidate cohort. The two administrations differ sharply in size, which is why you should never average them into a single number or call either one "the 2026 pass rate."
| Administration | First-time | All candidates | Repeat candidates |
|---|---|---|---|
| Spring 2025 | 74.4% (32/43) | 70.1% (54/77) | 64.7% (22/34) |
| Fall 2025 | 92.0% (242/263) | 89.4% (253/283) | 55.0% (11/20) |
Three honest takeaways: the fall cohort was far larger than the spring cohort; first-time candidates outperformed repeat candidates in both administrations; and these are PBSE figures only, not CAPE statistics. Small denominators (43 first-timers in spring, 20 repeaters in fall) mean percentages swing easily. See PBSE Pass Rate 2026: What the Data Shows for context, and How Hard Is the PBSE Exam? Complete Difficulty Guide 2026 for how to interpret difficulty beyond one statistic.
What Comes After PBSE
PBSE is an examination, not a standalone renewable credential. Maintenance requirements apply only after both exams are passed and the CCP is awarded. They are not additional PBSE domains and will not appear on the exam.
CCP Maintenance (Post-Certification)
Standard cycle requirements per the live ABCP recertification pages:
- 40 clinical activities annually, including at least 25 primary and no more than 15 secondary.
- 45 CEUs per three-year cycle, including at least 15 Category I CEUs.
- For new CCPs certified in the fall, the first reporting cycle requires 20 activities (at least 13 primary, no more than seven secondary); the standard 40 resumes afterward.
- Professional-activity CEU requirements are not prorated for the first cycle.
- Reporting periods end June 30; reports are due July 31.
A note on sources: ABCP's Booklet of Information shows conflicting editions (a parsed 2026 version 2.0 and a rendered 2025 version 1.2), so the figures above follow the live recertification pages rather than that booklet.
On the career side, no PBSE-specific salary premium was verified. Perfusionist pay depends on the occupation, call pay, and your employer, and the training investment and eventual CCP are separate considerations from the exam itself. For a careful treatment, see PBSE Salary Guide 2026: Complete Earnings Analysis and Is the PBSE Certification Worth It? Complete ROI Analysis 2026.
Scheduling Domains Across Your Weeks
One sequencing idea, tied directly to the domain structure: front-load the mechanism-driven science, then move to equipment and emergencies, and finish with integration under block-style timing. Adjust the length to your own calendar; the broader approach is in PBSE Study Guide 2026: How to Pass on Your First Attempt.
Foundational Science
- Domain 1 (Anatomy & Physiology) and Domain 3 (Pathology) first, since later domains build on them.
- Domain 2 (Pharmacology), focusing on mechanism over memorization.
Measurement and Systems
- Domain 4 (Laboratory Analysis) and Domain 11 (Monitoring) together, since both are interpretation skills.
- Domain 5 (Quality Assurance) and Domain 6 (Devices & Equipment).
Application Domains
- Domain 7 (Clinical Management), Domain 8 (Special Patient Groups), and Domain 9 (Special Procedures/Special Techniques).
- Domain 10 (Catastrophic Events & Device Failure), reasoning through failure scenarios from first principles.
Block-Format Rehearsal
- Timed mixed-domain sets in blocks, practicing final answers before submission.
- Review weak domains using domain feedback from official practice exams, plus additional drills at our practice test hub.
Frequently Asked Questions
No. PBSE is Part I of the ABCP's CCP process. The CCP is awarded only after you also pass CAPE, the separate Part II examination.
The exam has approximately 220 multiple-choice items with a four-hour testing limit. The item count is approximate, so do not rely on an exact figure. Starting Fall 2026, total time and approximate item count are unchanged, but delivery is in blocks you cannot revisit once submitted.
A scaled score of 400 on a 120-480 scale, using a criterion-referenced standard. There is no curve and no published raw-percentage cutoff. A preliminary result is given at the test center, with final verified results within six weeks.
No official percentage distribution was verified, so no domain can be called the largest. All 11 knowledge-base headings may be tested on PBSE, which emphasizes basic-science processes.
ABCP permits unlimited attempts, once per examination window, though separate state licensure attempt limits may apply. Retesting candidates may also be eligible for the January and July 2027 pilot windows if they have a prior-window application on file. Check the exam dates guide for window details.
